• 10 Customized membership certificates, high quality paper with 24lb thickness and watermark.
• Heavy duty cover set with snap-lock spine. Entity name label affixed to spine.
• Plain minute paper only.

*When filling out this order form a "*" to the left of the option indicates Required Fields listed below   in the partnership kit order form.
*Type Name of Entity exactly as you wish it to appear on the Membership Certificates.
*Year only applies for your corporate kit, except for California and Hawaii which require the Month   Day, and Year.
*Signature Titles are listed in ranking order of the officers of the Company. The highest ranking   officer is placed on the bottom right of the Membership Certificate, followed by bottom left, top   right and top left.
