Order Form Instructions   (Copy Protected Certificates)
* When filling out this order form a "*" to the left of the option indicates Special Instructions listed below.
* When using State of Entity (other), Name of Incorporation, Par Value (other)(i.e. $1,000.00) & Class (other) type    exactly as you wish it to appear on the Stock Certificates.
* Mo. Day, Yr. required for CA and HI only; all other States Year only applies.
* When typing Total Authorized Shares use numbers only (i.e. 10000).
* When using Numbering (other) list the to and from (i.e. 10-30)
* Signature Titles are listed in ranking order of the officers of the Corporation. The highest ranking officer is placed    on the bottom right of the Certificate, followed by bottom left, top right and top left.

Copy protection certificates
What is copy protection certificates?
When a copy protection certificate has been photocopied, a certificate with "void" on it will be printed. We currently only have Standard Certificates and Eagle Certificates in blue and green that have this feature.
